- Request an evaluation with MyMedicine if you are an Ohio resident and suffer from a condition which qualifies for medical marijuana therapy.
- If you have never used MyMedicine to obtain or renew your card, complete the virtual Intake Form with government issued identification on hand.
- Upload or email any medical documentation confirming your qualifying condition (first-time MyMedicine patients).
- After a quick review, you will receive a booking link by email to schedule your appointment and enter payment information. The charge processes after your appointment is confirmed.
- At your scheduled time, click the MyMedicine appointment link provided by email or text to initiate your telemedicine evaluation. If technical difficulties arise, the physician may give you a call so have your phone nearby.
- Your new or renewal recommendation is submitted to the State Registry by the provider shortly after your evaluation.
- Login to the State Registry at ohiomedicalmarijuanaregistry.com. After completing the required acknowledgment, download or take a screenshot of your Ohio medical marijuana card and head to one of many operational Ohio dispensaries.
MyMedicine offers a quick and easy process to schedule your evaluation for an Ohio medical marijuana card.
- Request an electronic intake form from our website. Upon submission and review of your form, a booking link will be provided by email. Returning MyMedicine patients do not need to submit an intake form.
- Select a day and time for your evaluation through the MyMedicine website. Billing information must be entered, but your credit card will only be charged upon appointment confirmation.
- At your scheduled time, click the appointment link to initiate a telemedicine evaluation, or the physician will call your phone number on file should technical issues arise.
Under Ohio law, qualifying medical conditions include all of the following:
AIDS, amyotrophic lateral sclerosis, Alzheimer’s disease, cachexia, cancer, chronic traumatic encephalopathy, Crohn’s disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, Huntington’s disease, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson’s disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, Spasticity, spinal cord disease or injury, terminal illness, Tourette syndrome, traumatic brain injury, and ulcerative colitis.
As of February 2021, the State Medical Board of Ohio determined the following conditions are considered covered by a qualifying condition: arthritis, chronic migraines, and complex regional pain syndrome.
After your telemedicine appointment, MyMedicine completes your recommendation. An email will be sent by the State indicating your recommendation is active and you may complete the State acknowledgement to activate your card.
The telemedicine appointment allows MyMedicine to meet all state guidelines prior to issuing a recommendation for medical marijuana. Shortly after your appointment, your card is available for use and can be accessed in the Patient Registry.
When searching for Registry emails, make sure to check your “Junk” or “Spam” folder if you cannot find it in your main inbox.
- Telemedicine – The implementation of telemedicine for medical marijuana patients in Ohio has lowered the cost of offering physician services. Without the overhead of maintaining offices, savings are passed to our patients.
- Value for Service – After receiving a medical marijuana card, dispensary staff guide patients through product selection. Our cannabis physicians confirm a qualifying condition, but do not provide specific product recommendations. At MyMedicine, we believe that the service we provide is appropriately priced based on this care model.
Due to the state-specific regulatory environment of medical marijuana programs and physician licensing, at present time we are only able to offer card services to Ohio residents. Residency is confirmed by government-issued ID, utility bill, or bank statement.
Before an Ohio medical marijuana card can be activated, MyMedicine must confirm a diagnosis of a qualifying condition.
If available, medical documentation confirming a diagnosis can be provided to MyMedicine in one of three ways:
- Upload documentation on the MyMedicine electronic intake form (BEST)
- Email a copy of your record to support@mymedicinellc.com
- Have your physician’s office fax your record to 833.563.2270
An image of the record should include your name, diagnosis, date, and the originating physician or institution. Patient portal images, pdf attachments, and scanned documents are all acceptable.
Many diagnoses may fall within the umbrella of a qualifying condition. Our office staff will review intake forms upon submission and inform you prior to the appointment if there are any issues confirming a qualifying condition.
The Medical Board requires all offices to keep records on file in a HIPAA-compliant manner for three years. Our staff is comprised of licensed healthcare professionals trained in HIPAA practices and all software utilized to retain records has met all HIPAA certifications.

An image of your government-issued ID is necessary during the intake process to ensure proper registration.
- The accuracy of the information electronically submitted to the state must match the physical ID. If not, you will not be able to be accessed in the Registry at the dispensary for entry.
- The address entered into the registry is based upon the address listed on your ID, not where you may reside at present time.
- A US passport, Driver’s License, or Identification Card are acceptable.
- Out-of-state ID’s must be accompanied by proof of residence in Ohio such as current utility bills or bank statements.
Patients registered with Veteran or Indigent Status may receive a discount on products at dispensaries. In order for your medical marijuana card to show this status, documentation must be provided to the state.
You must submit documentation separately after your evaluation. Please be aware that if either of these special status are selected, the state must first approve your documentation.
Each medical marijuana patient is allowed (2) Caregivers to make purchases on their behalf. If you would like to assign a caregiver to your account, complete the required information on the MyMedicine intake form. Please be aware that if caregivers are selected, the state must complete a background check before the Caregiver can be activated. The process may take up to 7 days.

MyMedicine evaluations are available by telemedicine only. A link to enter the telemedicine appointment will be sent by email upon confirmation.
Telemedicine provides for increased access to medical services through interactive communication with a computer, tablet, or smartphone. Telemedicine also lowers operating costs which allows us to pass savings to our patients.
During an evaluation, MyMedicine physicians ask and document answers to a few state-required questions regarding medical marijuana. These questions will relate to your medical history.
New patients receive an introduction on medical marijuana therapy, the product types available, and an overview of the Ohio Medical Marijuana Control Program if requested.
Renewal patients receive an assessment of medical marijuana effectiveness and discuss any changes with their qualifying condition.
MyMedicine physicians may provide general medical advice if it falls within their area of expertise and use clinical judgement on when to refer you to your regular physician or specialist.
MyMedicine physicians are able to advise on cannabis-related questions as it relates to your medical condition. Specific product recommendations are deferred to dispensary staff or MyMedicine pharmacists.
